RN Nurse Case Manager (Hospice)

Come join a team of seasoned nurses at Care Partners, one of Oregon’s oldest not-for-profit hospices! For over 40 years, we have cared for hospice patients across the Portland Metropolitan area, bringing interdisciplinary and holistic hospice services to patients and their support systems.

Care Partners is seeking a?full time Registered Nurse for the position of RN Case Manager?to provide coverage in the Portland metro area.

This position has candidates in the field providing direct patient care. We are a trusted name amongst our community partners, and receive consistent feedback from families that our nurses go above and beyond expectations to make them feel supported at their loved one’s end of life.

The onboarding and training process will involve hands-on learning with an RN preceptor, directly supported by our Director of Nursing, and will be tailored to your level of experience until you feel comfortable working independently

Schedule :

Schedule : Full time, days variable

Responsibilities :

Assesses, evaluates, and regularly reevaluates needs of the patient and family, organizing resources to meet those needs

As case manager, coordinates the care provided by all hospice disciplines.

Provides nursing care as needed; attentive to physical, emotional, and spiritual needs of patient

Counsels patient and family regarding symptom control, medications, direct nursing care measures, nutrition, etc.

Assists patient and family in making choices that best meet their needs by providing alternatives and support

Attends deaths, supports families, assists with final arrangements

Admits new patients, educates families, arranges services and equipment, obtains physician orders, and procures medications

Collaborates professionally with other health care providers, including the patient’s physician, pharmacies, and care facilities to ensure optimal care for patient

Communicates often and well with other members of the interdisciplinary team; functions as an integral member of the patient care team

Develops and revises, as appropriate, patient care plan in collaboration with other IDT members, completing the necessary documentation and attending IDT meetings for assigned patients

Provides all services in full compliance with Medicare regulations, including timely, professional documentation. Documents all patient family contacts and interventions and maintains accurate and up-to-date patient records and care plan including observations and reports of patients eligibility for Medicare Hospice Benefit

Be a part of the night nurse and weekend float pool.

Qualifications :

Requires current unencumbered Oregon licensure as a Registered Nurse prior to first day of employment

Prefer candidates with one year of professional nursing experience in hospice, palliative care, home health, medical surgical care or equivalent

Must have a valid driver’s license and acceptable driving record and have access to a reliable automobile to travel to and from patient homes

Requires well-developed critical thinking, patient assessment, and problem-solving skills

Requires the ability to work independently and as part of an interdisciplinary team with strong verbal and written communication skills

Must have good skills in using electronic tools such as email, internet, keyboarding and be able to learn medical software to use for accurate and timely charting.

Bi-lingual, Spanish speaking ability and certification in Hospice and Palliative Care are helpful but not required

Requires basic life support CPR card or ability to obtain a valid card prior to patient contact within two weeks of hire.

Preferred applicant will be asked to pass a background check and drug screen following a job offer.

New employees are required to complete tuberculosis screening and provide proof of Hepatitis B vaccination or signed declination.
