Our Mission for Cleveland Clinic Hospice is attending to suffering and caring for patients and their families, by listening, partnering, and communicating with compassion.

Under the direction of the hospice manager, the Clinical Chaplain provides spiritual care to patients, families and caregivers fostering spiritual resources for healing.

Spiritual care is a component of healing, so meeting others where they are and helping them amid troubled waters is a vital part of the Chaplain's role.

Your daily direct patient care will be performed in various inpatient settings such as assisted living and skilled nursing facilities, as well as patients' residences.

The ideal future caregiver is someone who : Has experience working in hospice and in the hospital setting. Provides a calming, compassionate presence.

Is an active listener. Demonstrates religious sensitivity. Is empathetic and team oriented. As the Clinical Chaplain, you will be able to attend and participate in monthly 'Today in Ethics' seminars that are focused on palliative and hospice medicine.

Responsibilities : Performs and documents a Pastoral Care assessment within five working days of initial referral, utilizing interview, observation and other available information.

Evaluates the patient and family need for immediate attention and appropriate action with each visit.Establishes a written patient plan of care on primary patients and documents intervention within 72 hours of visit.

Implements and follows the established plan of care with each visit.Revises / updates patient care plan at each visit / contact based on changes in patient's condition and evaluation of effectiveness of pletes required documentation within specified departmental guidelines, policies and procedures.

Demonstrates appropriate psychosocial intervention based on assessment data when working with age specific and high risk patient populations.

Assesses and documents the patients / families level of knowledge related to the patient's specific spiritual care needs with each visit.

Interprets the hospice mission, philosophy and policies to patients and families on initial visit and as needed based on specific needs.

Provides education to meet the patient's spiritual needs as identified by Pastoral Care and / or patient / family / or significant other based on patient's age and developmental status when appropriate.

Utilizes teaching methods that need the age specific need and knowledge level of the patient and family.Evaluates and documents the effectiveness of the education provided with each visit.

Other duties as assigned.Education : Master of Divinity or equivalent- RequiredSatisfactory completion of a minimum of four units of Clinical Pastoral Education- RequiredAdvanced knowledge / skills in spiritual and psychosocial assessment- preferredAdvanced knowledge of the dying process.

Advanced knowledge of the stages of grief and the grieving process- preferredLanguages : English RequiredCertifications : Certifications requiredCertification as a Board Certified Chaplain (or eligibility for same within two years, or certification as an ACPE Supervisor of Clinical Pastoral Education - RequiredCurrent Basic Life Support (BLS) Certification through the American Heart Association (AHA).

Valid Ohio Driver's license upon hire; per policy 8-175 thereafter.Proof of automobile insurance coverage with $100,000 / $300,000 coverage, upon hire;

per policy 8-175 plexity of Work : Within scope of job, requires critical thinking skills, decisive judgment and the ability to work with minimal supervision.

Must be able to work in a stressful environment.Advanced skill in grief counseling.Excellent organizational and communication skills, both verbal and writtenWork Experience : 1 year of ministry experience- requiredPrevious experience in hospice or home care or experience in working with the dying -preferredPhysical Requirements : Ability to sit or stand for extended periods of time.

Must be able to climb stairs, pull, push and facilitate movement of the patient.Must have normal or correctable vision and hearing to be able to hear normal conversations.
