Responsibilities

Ascension Wisconsin is looking for a Family Medicine Provider for our Bay View Clinic

This is a great opportunity for the right candidate to partner with Ascension Wisconsin to provide medical services to patients.

Position Highlights :

Join a well-established multidisciplinary practice consisting of :

o 3 Family Medicine Physicians and 2 Nurse practitioners for primary care

Onsite Cardiology and Electrophysiology

o 1 : 10 rotating weekends and weekdays (light volume of phone call, can expect less than 3-4 calls per weekend, no expectation of hospital admission or rounding) Ascension Connect RNs take the call first

• Average 18-20 patients per day
• General full spectrum outpatient Family Medicine practice

o Option to do Inpatient and OB if interested, but would be in call rotation

• Very loyal patient panel in quaint and diverse neighborhood community
• Busy Clinic; ability to build practice quickly
• Provide 36 patient contact hours per week including 2 non-traditional hours
• Must Be Board Eligible / Board Certified in Family Medicine or IM / PEDS

Clinic hours :

o Monday-Friday : 7 : 00 AM 5 : 00 PM

• Physical and Occupational Therapy on-site
• Onsite Lab

o EKG

o Spirometry

o Blood Draw

o POC testing

• Digital Radiology
• Epic EHR

Requirements

Licensure / Certification / Registration :

Physician MD / DO credentialed from the Wisconsin Medical Examining Board obtained prior to hire date or job transfer date required.

Verified by local Credentialing Office.

Advanced Life Support credentialed from the American Heart Association (AHA) preferred. Verified by local Credentialing Office.

Education :

• Doctor of Medicine (MD) or Doctor of Osteopathy (DO) required.
