Overview Responsibilities : Join an established Primary Care practice Perform patient assessment and assign diagnosis based on recorded assessment data and current scientific and current research.

Establish plan of care and implement interventions in accordance with care paths, protocols, standards of care and current research.

Provide education and counseling to patients, families and groups. Clinic hours are Monday Friday 8am-5pm Excellent work / life balance with no call or weekends.
