Joinour client's team as an Enrollment Registered Nurse for aProgram in Los Angeles. This individual will help to bring theprogram to serve frail elders and disabled older adults inunderserved communities across Los Angeles.

The EnrollmentRegistered Nurse is a key player of the Outreach Team as we findand assess prospective participants for our program.

This personacts as a liaison between the IDT (Interdisciplinary Team) and theOutreach Team to ensure a smooth enrollment process for theprospective participants.

KeyResponsibilities

Collaborate with OutreachSpecialist and IDT to ensure successful enrollment

Conduct home visits to evaluate the prospective participant forpotential eligibility for nursing home level of care

Conduct home safety evaluations of the prospectiveparticipants’ home

Participate in EnrollmentEligibility meetings to review Level of Carerecommendations

Complete Nursing Facility Level of Care(NFLOC) assessments and submit to DHCS for review; update / revisethe assessment as requested by DHCS

Maintain accuratedocumentation and record all evaluations in CRM software

Participate in all Audits by DHCS and CMS

Support theCenter Director to develop participant retentionstrategies

Maintain knowledge and expertise ineligibility enrollment and program specifications for theProgram

Support business development and census growthby participating and assisting with outreach programs asneeded

Schedule and Shift Details

MF 8a5p. Mostly infield home appointments and occasional onsitework in Los Angeles clinic up to 5 days per month.
