As a Per Diem Home Health Registered Nurse, you will :
- Conduct skilled nursing visits for home health patients, including admission, case management, and follow-up care.
- Collaborate with physicians and clinical managers to develop and implement patient care plans.
- Coordinate care with other healthcare personnel such as Physical Therapists and Home Health Aides.
- Report patient progress to physicians and Clinical Manager on an ongoing basis.
- Supervise and provide instructions to Home Health Aides and Personal Care Workers.
- Prepare and complete medical documentation for all patients.
Qualifications :
- Diploma, Associate or Bachelor Degree in Nursing
- Minimum of one year nursing experience preferred
- Current and unrestricted Registered Nurse licensure
- Current CPR certification
- Strong organizational and communication skills
- Valid driver's license, auto insurance and reliable transportation.
Compensation :
$76,800 - $105,800 per year
