RN Salary : starts at $85K / year + $2,500 Sign-On Bonus
LPN : Hourly + Pay Per Visit (PPV)
Service Area : South Central metro (coverage area : St. Louis Park, Richfield, Bloomington, Burnsville, Lakeville and Surrounding).
Plus mileage reimbursement of .67 cents per mile.
Schedule : Monday - Friday, 8 : 00am-5 : 00pm (with rotation every 6th weekend)

The RN / LPN Visit Nurse is responsible to plan, organize and direct home care services and is experienced in nursing, with emphasis on community health education / experience.
The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.
Qualifications :
- One to two years of recent acute care experience in an institutional setting. One year of home health care experience preferred.
- Experience in customer service / direct member contact field preferred
- Experience supervising caregivers
- Graduate of an accredited school of nursing
- Current Minnesota Nursing License or Minnesota Licensed Practical Nurse
- Must pass a MVR check based on Lifespark company policy
