McGregor PACE (Program of All-inclusive Care for the Elderly), is a community service program that provides in-home healthcare services to the elderly as an alternative to nursing home placement, enabling seniors to remain "at home".
CLINIC LPN : FULL TIME, MONDAY-FRIDAY, 1ST SHIFT ONLY
We are seeking an experienced Clinic LPN who will work closely with our Primary Care Physicians and as a key member of our Interdisciplinary team, to deliver comprehensive health services for frail / elderly program participants at risk for nursing home placement.
The position requires an energetic contributor who excels at working in a team setting.
Must be a self-starter. Team-oriented to perform assessments, acute triage and provide direct nursing services for frail elderly.
Must be detail oriented with excellent assessment, verbal and written communication skills.
Our ideal candidate will support the day to day clinical operations and provide direct nursing care to participants.
Qualifications :
- LPN licensed in the State of Ohio, required. Valid ODL; BLS certification preferred
- One year experience working with older adults in community health, LTC, or adult day care preferred.
- Prior experience with dementia care, desirable
- Previous experience in multidisciplinary care setting desirable
- Computer literate, organized, flexible and able to multi-task in a fast paced environment.
- Physical ability to perform job core job responsibilities
