Job Summary :

This position is responsible for providing case management and advocacy services under the King County Best Starts for Kids (BSK) Youth and Family Homelessness Prevention initiative (YFHPI) program at the Seattle Indian Health Board.

The Case Manager shall provide the support needed to ensure that unaccompanied youth / young adults (ages 12-24) and families at imminent risk of homelessness have access to the resources needed to avoid entering the homeless system;

and, to retain stably housed over time.

Responsibilities :

Provide client centered case management that addresses the immediate issue that is placing the family or youth at imminent risk of homelessness.

Case management services shall be delivered at the time and location most convenient for the client.

  • Using a progressive engagement model, the Youth Case Manager will provide services to those that are needed to achieve housing stability.
  • Mobile advocacy will be delivered at locations most convenient for client or families who are unstably housed, couch surfing or on the verge of eviction or homelessness by providing hotel vouchers, sheltering services, housing / rental applications and wrap around financial assistance.
  • Advocate and emotionally support clients and their families whom are in educational, legal, or other systems which impact their health and wellbeing.
  • Identify and utilize community resources necessary to ensure success for clients plans, e.g., child / youth services and health services, housing / shelter resources, gender relevant support networks, legal networks, job training programs.
  • Facilitate educational support groups for clients and / or their families that address homeless related topics.
  • Collect and report client-level data into the County's prevention data base. Complete reports based on grantors and SIHB requirements that are based upon accurate client data and submitted in a timely manner.
  • Partner with other organizations and agencies seeking to support homeless youth and families.
  • Participate in any / all one-on-one technical assistance opportunities that build agency / program capacity to deliver services.

Attend other internal and external meetings and / or trainings as required or directed.

  • Participate in internal and external meetings, training, activities and events as needed to provide high quality services for homeless youth and their families.
  • Comply with SIHB's policies and procedures.
  • Actively participate in internal quality improvement teams and work with members proactively to drive quality improvement initiatives in accordance with the mission and strategic goals of the organization, federal and state laws and regulations, and accreditation standards, when assigned.

Qualifications

Qualifications :

  • MSW Preferred
  • 3 years experience working as a Case Manager required.
  • Knowledge of health, social, and homeless issues and services faced among youth in King County is preferred.
  • Knowledge of health and social issues facing American Indians / Alaska Natives and a desire to serve this population.
  • Current WA State Driver's License is required.
