What is the ALF Resident Care Manager (RCM) Experience?
As the ALF (Assisted Living Facility) Resident Care Manager (RCM), you will provide oversight and direction for the resident health care functions to ensure high-quality care and a positive working environment.
Your responsibilities will include assessing residents' needs, coordinating care plans, and fostering positive relationships with residents, families, and staff members.
You will conduct thorough evaluations of residents' physical and mental health, family and community resources, and the physical environment.
Ensuring that pre-placement screenings are completed to assess the appropriateness of placement and readiness to meet resident care needs prior to admission will be part of your role.
You will collaborate with interdisciplinary teams to develop, implement, review, and revise individualized care plans / service plans for each resident, ensuring assessments are conducted regularly and whenever there is a change in condition.
Additionally, you will oversee resident medication administration, maintain accurate medical records, and delegate nursing tasks appropriately.
Building strong relationships with residents, families, staff members, and supporting functional groups through proactive communication and feedback is essential.

What Are the Job Qualifications?
We are looking for a Registered Nurse in the state of Oregon, with a BSN preferred. You should have a minimum of 2 years of nursing experience, preferably in a long-term care setting, though this is not required.
At least 1 year of supervisory experience is necessary. You must be able to interact well with staff, residents, and families, and possess strong time management, organizational, and project management skills.
