Licensed Practical Nurse Primary Care Plaza

Job Description

We are hiring a full time Licensed Practical Nurse for our Primary Care - Perryman Clinic at the Plaza.

The Schedule :

This is a Monday through Friday day shift position.

The Work :

LPN will assist with patient phone calls, procedures, rooming, and various administrative tasks as needed (ex : prior auths, RX refills, etc).

• You will be working with a variety of providers and their patients, working in a blended role.
• Provides phone triage, assisting with rooming patients, downloading blood glucose meters and pumps, and educating patients on injectable medicines and glucose meters.
• Because you will not be assigned to one provider, the ideal candidate will be flexible and able to adapt to different provider preferences.
• We are looking for a strong team player who is wiling to jump in and help when they see a need.
• Previous experience with blood glucose meters and pumps is preferred but not required.
• Provides support for assigned providers.
• Completes patient care, patient education, prior authorizations, in basket messages,, prescription refills, and other duties as assigned.
• Responsible for telephone triage, patient flow, and coordination of patient care
• Schedules emergent need patient appointments
• Identifies learning needs for patients and families, provides advice and recommendations, and inserts intravenous access with medication administration.
• Delegates tasks to clinical staff in accordance with demonstrated competencies and consideration to Scope of Practice
• Enters admission orders as directed by provider
• Manages expired samples.
• May be responsible for covering Medical Assistant job duties or other office duties as needed

The ideal candidate will be highly organized, task oriented, resourceful, and possess strong communication skills.
