Direct Support Professional

Summary :

The Direct Support Professional : is responsible for providing quality services per Outcome measures (ensuring health and safety, enabling people to make decisions, assisting people to understand and exercise their rights, etc.

directly to individuals with developmental disabilities to help them achieve their goals.

This position is responsible for providing direct support, positive direction, and assistance to individuals in accordance with program policies and procedures;

and personal outcome measures.

Detail of Duties and Responsibilities :

  • Provides supports to people served by assisting in the development and implementation of all Habilitation Plans, including spiritual life supports, assisting people in achieving their personal goals and desires, providing interaction and choices for activities that support a meaningful day, and supporting people to be independent in activities of daily living;
  • Assists with information gathering for Personal Outcome Measures, and other assessments; Assists people with maintaining a clean, safe, and orderly home by following safety and infection control procedures;

educates people about maintaining homes; Maintains and expands skills and knowledge relevant to providing high quality support and services.

  • Obtains and maintains all required certifications and trainings and applies the information to support people served; Maintains open communication and cooperative work effort with Lead staff and other staff to assure quality and continuity of support to people served;
  • Supports the Human First Mission, Vision, and Core Values while empowering each person to direct their own life choices;

Reports and / or documents routine and unusual events including completing daily logs, accident reports, outcome data sheets, etc.

  • in a concise, understandable, and legible manner; Assists people with fiscal management while safeguarding their funds. This may include completing ledgers, reconciling accounts and assisting people with spending choices;
  • Ensures that people exercise their rights and that rights are not restricted without due process, ensures people are treated with dignity and respect;
  • Responsible for ensuring health objectives for individuals including following doctor’s orders and dispensing medication;
  • Responsible for maintaining a clean and therapeutic environment; Support people to complete necessary grooming / hygiene including, but not limited to, assisting in restroom, with meals, and with dressing;

Ensure people’s rights are respected; Know the goals, interests, and needs for assistance of the individuals you support Transport individuals as needed to appointments, leisure activities, etc.

  • Follow written plans (including positive behavior plans) to systematically teach people new skills and document progress according to the schedule identified in the plan or by the supervisor;
  • Identify and look for solutions to potential barriers to individuals’ success Provide feedback to the team about community events and other issues / concerns;
  • Complete all Agency Core Trainings and other duties as assigned.

Physical Requirements :

This position requires seeing, hearing, standing, walking, bending, the use of stairs, lifting, carrying supplies, communicating both verbally and in written form, the ability to make sound and immediate decisions in order to ensure the safety of the people we support.

Regular attendance is mandatory. Must be physically able to provide necessary first aid / emergency medical treatment, provide physical support to individuals such as, assisting with personal needs, etc.

and to physically evacuate individuals from living arrangements in case of fire / natural disaster. Must be able to transport individuals in their community.

Must be able to use computer software to complete essential functions listed above. Experience and Education Qualifications : High school diploma / GED required.

One year experience in a human service field preferred. Some college coursework in human services is preferred. Ability to achieve SCIP-R (Promote), CPR / First Aid / Medication Certification and Valid NYS License required.
