Geriatric Primary Care Physician, Community Health Center (FQHC)
Federally qualified community health center in seeking a geriatric physician to join the health centers medical staff. The candidate should be comfortable taking care of older patients with multiple comorbidities.
The Geriatric Primary Care Physician Creates an ongoing relationship with each patient by providing access, continuity, education and comprehensive care via diagnosis and treatment of patients at the health center.
Actively supports organization priorities of patient care, quality, education, and sustainability for elder adult. Provides patient centered service and supports a positive work environment.
Provides for the patients health care needs and when needed arranges care with other qualified professionals. Assures quality and safety by adhering to PCMH principles, including teamwork, evidence-based medicine, CQI, and active participation of patients in decision-making.
Provides timely follow-up on patients seen including chart notations, lab review, and referrals to specialists including providing after hours coverage as required by site.
Provide on-site day to day clinical supervision and back up of Family Nurse Practitioners and Physician Assistants as needed.
Provide back up for Family Physicians as needed. Perform chart reviews and give feedback to PCPs as part of ongoing quality assurance in geriatric care.
The candidate will have the opportunity to with a multi-disciplinary team of physicians and support staff including, nurses, social work, clerical and administrative staff.
In addition, he / she will have the collaborative support of all the other providers. Must have excellent clinical and communication skills and work well in a team.
