Job Title : Registered Nurse (RN) - Home Health Job Specialty : Continuing Care Job Duration : 13 weeks Shift : Day (0700-1900), every other weekend Guaranteed Hours : 40 hours per week Experience : Minimum of 1-year home health experience License : Current RN license or temporary license as a Registered Nurse Petitioner in the state of practice or in the primary state of residence if residing in a National License Compact (NLC) state.
Certifications : Current Basic Life Support (BLS) for Healthcare Provider (American Heart Association) Must-Have : - Home health experience with wound care, pleurx, infusion, venipuncture, and case management - Experience with Start of Care (SOC), Therapy, geriatric / pediatric, chronic disease management, maintenance, and OASIS-E - Effective communication skills in English (reading, writing, speaking) Job Description : - Collect data pertinent to the healthcare consumer's health or situation - Analyze assessment data to identify actual or potential diagnoses, problems, and issues - Identify expected outcomes for an individualized care plan - Develop a plan to attain measurable outcomes - Implement the plan, coordinate care delivery, and promote health and safety - Delegate care to appropriate healthcare workers within legal or policy guidelines - Evaluate progress toward goals and outcomes - Practice in alignment with cultural diversity and inclusion principles - Communicate effectively across all areas of practice - Collaborate with healthcare consumers and stakeholders - Demonstrate leadership in professional practice - Seek knowledge and competence in current nursing practice - Integrate evidence and research findings into practice - Contribute to quality nursing practice - Evaluate own and others' nursing practice against professional standards - Utilize appropriate resources for safe, effective, and fiscally responsible care - Practice in an environmentally safe and healthy manner - Maintain ethical practice - Promote shared governance and decision-making activities, fostering research to improve clinical outcomes Physical Requirements : - Ability to walk, stand, sit, lift, reach, stoop, bend, push, and pull - Must be able to lift and support up to 35 pounds - Intact sense of sight, hearing, smell, and touch;
