8 : 00 a.m. to 4 : 30 p.m.
Can be flexible with work schedule and work either 4, 9 hour days or 5, 8 hours days
No routine On-Call requirements. May only be asked to cover vacations or during staff turnover.
Job Summary
The Home Health and Hospice Registered Nurse (R.N.) is a professional nurse who provides quality, cost effective, client-centered, skilled nursing care to support, maintain and implement Home Health and Hospice Care.
Initiates action within limits of responsibilities and according to established policies and procedures. Case Management of Assigned client is performed.
Assures privacy and confidentiality for all patients / families, adheres to departmental and organizational policies.
Assessment
- Makes the initial evaluation visit to a client that has been referred for services.
- Admits the patient using the OASIS document for Home Health patients and the Comprehensive Assessment document for Hospice patients.
- Demonstrates excellent assessment skills to complete the initial and subsequent evaluations of patients.
Planning
Establishes individualized nursing plan of care based on patient’s needs.
Implementation
- Implements individualized plans of care according to established policies and medical orders.
- Ensures documentation is accurately completed within expected timeframe per agency policy.
- Periodically revises the plan of care based upon changes in assessment data and / or client needs.
- Assists in the maintenance of a safe, comfortable and therapeutic environment for the client.
- Applies therapeutic communication skills to interact with patients and families.
Evaluation
Evaluates patient outcomes compared to goals of care and plans for discharge or care plan revision as appropriate.
Team member
Communicates effectively and collaborates with each member of the care team to achieve excellent patient outcomes (care team members may include discharge planners;
physicians; other nurses; therapists; hospice social worker, chaplain, bereavement coordinator and volunteers; and staff at facilities where home health and hospice patients may reside).
Professionalism
Demonstrates professional attitude towards self, client, families, healthcare team members and community resources.
Customer Service
Demonstrates excellent customer service to all internal and external customers of the home health and hospice agency.
Service
- Maintains all equipment provided by Holton Community Hospital
- Assists with routine department tasks to help keep office running efficiently.
- Promotes the mission and values of Holton Community Hospital in all interactions, represents the hospital with pride and professionalism.
- Other duties as assigned by the clinical manager or director of community services.
Requirements
Education
Must be a graduate from an accredited School of Nursing. BSN preferred.
Experience
Three (3) or more years in clinical experience with demonstrated abilities. Experience in Home Health / Hospice / Community Health desirable.
ER or ICU experience preferred.
Certificates, Licenses or Registration
- Current license issued by the Kansas State Board of Nursing or Compact State License.
- Current BLS certification
- Valid Kansas Driver’s License and Proof of Automobile Insurance
Language Skills
- Must be capable of communicating clearly (reading, writing and speaking) in the English Language.
- Must be capable of reading and comprehending written policies / procedures and operation / maintenance instructions. Must have the ability to receive and relay phone or other business-related messages
Equipment
- Must be able to operate computer, telephone and other department specific equipment required to perform your job.
- Basic computer skills necessary (Basic skills in Word, Outlook, and Excel are essential).
- Experience with documentation in an electronic medical record preferred.
