POSITION SUMMARY :
To be fully engaged in providing Quality / No Harm, Customer Service and Stewardship by utilizing the nursing process to improve outcomes, to assure appropriate resource utilization, and continuity of care for individuals with altered skin integrity and other wound care issues.
This includes coordinating the care and services for assigned patients in collaboration with the patient, family, physician, patient care team, and payers.
Advanced clinical skills will be utilized to facilitate the provision of care which includes the assessment, planning, intervention, and evaluation of patient care.
This care is based upon cultural, spiritual, and age specific needs of the patient.

Associates support engagement through adherence to the (I?CARE) commitments.
Quality / No Harm :
- Demonstrates effective and safe clinical practice as evidenced by skills fair demonstrations, completion of yearly competency assessment and age specific checklist.
- Coordinates inter?disciplinary plan of care based on current evidenced based practice. Assesses patient / family and collaborates with physicians and interdisciplinary team to identify clinical needs.
- Coordinates the referrals of home health care, home medical equipment, and other services as appropriate in a timely manner.
- Documents all nursing care, change in condition, response to therapy, treatments, procedures, and referrals on patient record to meet standards.
- Demonstrates dependability, reliability, and flexibility.
- Demonstrates appropriate delegation and supervision of patient care activities within the health care setting.
- Demonstrates effective communication with other members of the health care team and promotes effective team functioning.
- Participates in orientation of new associates.
- Serves as clinical preceptor to undergraduate nursing students.
- Proposes alternative treatment options as appropriate to ensure a cost effective and efficient plan of care and quality patient outcomes.
- Follows through with referrals from physician for consultations as indicated.
- Demonstrates ability to recommend appropriate nursing interventions and alternatives in the care planning process.
- Possesses a working knowledge of available community resources.
Stewardship :
- Uses and cares for equipment and supplies economically.
- Contributes to the development, implementation, review of standards of care, and policies / procedures related to patients with wound care needs.
- Assumes leadership role per needs of facility.
- Obtains insurance authorizations as needed for advanced wound care modalities.
- Participates in research to improve practice and outcomes (eg. Wound healing rates, product evaluation, etc). Takes and active role in coordination of prevalence and incident studies per needs of the facility.
Customer Experience :
- Provides patient, family, and / or care giver education as directed by the plan of care.
- Provides service to patients and families with sensitivity and respect for their needs, expectations, age, cultural, and individual differences.
- Contributes to the patient experience as evidenced by positive patient experience scores and / or written compliments by patients / others.
- Prompt response to patient care per physician orders.
- Responds and anticipates patient / family needs while in the facility.
- Demonstrates ability to relate to and work with ill, disabled, elderly, and emotionally upset patients and families within the facility with compassion and professionalism.
Qualifications
QUALIFICATIONS REQUIRED :
- Registered Nurse with current Florida license.
- Must hold current AHA BLS Health Care Provider completion card prior to start date and to maintain.
- At least 3 years clinical experience preferred.
- Wound Care experience strongly preferred.
PHYSICAL DEMANDS :
- Must have visual acuity and hand?eye coordination to perform clinical tasks.
- Must have corrected vision and hearing to normal range.
- Must be able to lift, turn, pull, push, or ambulate patients / equipment up to 90% of the assigned shift.
- Must be able to lift up to 40 pounds unassisted up to 33% of the assigned shift.
- Must be able to stand, walk, bend, and stoop up to 90% of assigned shift.
- Potential for exposure to bio?hazardous waste and unknown or known diseases.
MENTAL DEMANDS :
