About The Position

LTC or Rehab experience requiredLicense : Active State Licensure / Registration where applicableCertifications : COTA license requiredMust-Have : Experience in LTC or Rehab settingsJob Description : - Utilize evidence-based practice to support clinical interventions.
- Provide rehabilitative, skilled, and medically necessary treatment interventions to patients with cognitive and / or physical functioning disorders under the supervision and direction of an Occupational Therapist.
- Administer various procedures as part of the rehabilitation plan, including ADL retraining, therapeutic activities, therapeutic exercises, modalities, and the use of supportive and assistive devices.
- Assess patient progress and communicate with the supervising Occupational Therapist to adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
- Report the effectiveness of treatment outcomes to the supervising Occupational Therapist.- Offer consultation to patients, families, caregivers, and other service providers related to cognitive and / or physical disorders.
- Communicate effectively with supervisor and other health team members regarding patient progress, barriers, and treatment plans.
- Provide input to supervisor for discharge summary of services provided per regulatory and clinical requirements.- Document all regulatory and clinically required information in the electronic documentation system and maintain patient records according to company policy.
- Accurately record treatment minutes for services provided per all regulatory and clinical requirements.- Participate in required meetings including patient care conferences, utilization review meetings, and rehabilitation meetings.
- Participate in facility and company-required training, in-services, and conferences.- Work collaboratively with the supervising Occupational Therapist to obtain necessary DME or supplies per regulatory and clinical practice requirements.
- Supervise and support students under the guidance of the Occupational Therapist.- Adhere to NBCOT / AOTA and State Required practice acts, code of conduct, and ethics during professional practice as an Occupational Therapy Assistant.
