Input on patient-centered models.Community focusClinical Care (1) The primary responsibility for ambulatory primary care is at the assigned clinic location which includes primary care, addiction, mental health, and chronic care delivery.
2) Review client records for needs of medications, treatments, education or referrals. (3) Perform comprehensive health assessments of clients by completing a physical and psychosocial history.
4) Identify actual and potential health needs based on medical, mental / emotional, financial, and social factors. (5) Prescribe and dispense medications.
6) Initiate and / or continue medical regimes for new patients as well as chart treatment records and monitor results. (7) Establish appropriate treatment plans for patients.
Consultation (1) Provide clinical consultation to other providers, healthcare staff and supervisors. (2) Provide educational expertise and evaluate care within the facility.
3) Participate in collaborative treatment with other medical and mental health professionals and peer review. Education / Training / Experience : (1) Graduation from an accredited school of Medicine AND One (1) year residency in a primary care setting Certification : (1) Possession of, or ability to obtain, an appropriate and valid license to practice medicine in the State of Oregon and ability to obtain credentialing approval.
