Job Summary
Under the general supervision of the RN supervisor, as well as the DON, the LVN Charge Nurse is responsible for representing St.
John of God Retirement & Care Center and specifically the residents of SNF, in coordination, assessment, admission, med-pass, and oversight of CNA / RNA staff, and delivery of personal services for those residents.
Also, included in those responsibilities is the supervision of all assigned nursing personnel. This supervision shall include residents of floors and nursing stations (as assigned).
Breaks may occur where an LVN must cover multiple stations for limited times. Lunch and 15 minute breaks must be taken during medically and policy appropriate times, in coordination with other LVNs and the RN Supervisor’s knowledge.
Breaks will not be denied without compensation (on duty meal break form must be filled out, or otherwise informed to payroll).
Competence in use, basic maintenance, training others, and accuracy must be shown with regard to the Nursing Department’s Software.

Essential Functions
- Knows and uses accurate information concerning the policies and procedures which govern divulging of information, the location and function of various departments, and the names of appropriate supervisory personnel
- Be present, systematic, and responsible in referring any inquiry or problem regarding family, resident, or visitor to the appropriate staff person
- Knows and implements all Department of Social Services regulations regarding medical assessment, resident records, personal rights, incidental medical and dental care, personal assistance and care, functional capabilities, and reappraisals, prohibitive health conditions, and general requirements for allowable health conditions
- Delivers and supervises quality resident care
- Interacts with all residents, families and staff in the spirit of St. John of God
- Coordinates resident interests and needs with the staff team
- Maintains proper surveillance of medications and records, as required by regulatory agencies 8. Ensures that physicians’ orders are carried out
- Facilitates ordering and distribution of resident medications
- Monitors overall wellbeing of the resident, communicating with resident, family, and physician, reporting any changes in condition to physician
- Performs all nursing duties according to nursing standards
- Communicates all changes of condition to ED for Residential Services
- Be present at major activity programs when possible
- Supervises CNAs, RNAs and LVN Charge Nurses
- Assists in clinical and CNA / RNA work when needed, informs DSD in written report of any employee incident needing further investigation
- Schedules and monitors CNAs, RNAs, and LVN Charge Nurses’ assignments
- Documents personnel counseling and performs write-ups when necessary
- Performs annual evaluations of personnel in a conscientious, timely manner as directed by the Supervisor or DON
- Directs all nursing policies and procedures
- Evaluates incoming patients and ensure evaluations are done in a timely manner
- Maintains a peaceful and professional environment, informs management of potential disruptions to the professionalism of the work environment
Additional Responsibilities & Duties
- Attends required in-service training programs and implements what is learned
- Prior to admission, review resident’s social and medical history, ensures assistance is available as possible when an admission arrives
- Plans and coordinates a weekly team Service Plan review as needed
- Reviews a resident’s care, or when there is a change of condition of residents
- Communicates with all disciplines to ensure care is carried out
- Keeps current all doctors’ orders for residents
- Maintains the residents’ Medication and Treatment Delivery Schedule
- Advises the nursing staff of any information needed for the resident’s care
- Maintains schedules for weights and baths
- Notes any change in resident in shift report for the nursing staff
- Reviews caregivers’ shift reports daily
- Conducts a daily tour of resident rooms
- Be present to residents during meal times, and supervise the serving of special diets
- Maintains all required forms in resident charts
- Arranges for transportation to medical appointments, if needed
- Performs other duties as assigned
- Follows organizational and departmental procedures

Physical Demands & Working Conditions
- Able to handle pen / pencil and telephone receiver
- Uses fingers for keyboard at least 2 hours a day
- Be physically able to assist a resident in a reasonable manner during the likely scope of daily work
- Must be able to assist residents out of the facility as needed (down stairs, carrying heavy loads)
- Assists in lifting, pushing, or pulling residents when / if needed (at minimum 50lbs for short spans, multiple times per day, maybe up to 75lbs with assistance for short movements)
- Needs to walk a minimum 500 feet at a time, several times a day, must be able to walk up and down stairs in event of power / elevator outage or emergency and assist residents in doing the same
- Lift up to 50lbs : Occasionally to carry medical charts, supplies and resident’s personal items, e.g.
clothing and food items; Occasionally when lifting soiled bed linens; occasionally to frequently when assisting an ambulatory resident in bathing or dressing
- Reach above shoulder height : Rarely, occurs when reaching for medical records or supplies located on shelves
- Occasional grasping, squatting, kneeling, twisting, bending, or kneeling when assisting nursing staff with resident care
- Must follow all safety measures and procedures
- Must follow the dress code policy
Required Qualifications
- Current LVN License
- Current CPR, First Aid, Fire and Life Safety Certificates
- Graduate from an accredited School of Nursing
- Some nursing experience required
- Strong interpersonal skills

- Must have ability to develop and maintain a good working relationship with all facility personnel, physicians and paramedical professionals, governmental agencies and their representatives, and community
