The Home Care Physical Therapist evaluates and develops treatment plans for patients of all ages, modifying the plan of care as appropriate.
Serves as a role model in the delivery of professional services and as a clinical resource for staff and students. Hours may vary depending upon census and program need.
- Completes comprehensive assessments of patients and develops individualized treatment plans.
- Educates patients and their families on home exercise programs and home safety measures.
- Collaborates with healthcare professionals and communicates regularly with the care team to ensure effective patient care.
- Documents patient visits thoroughly and ensures all necessary paperwork is completed accurately and on time.
- Participates in discharge planning and ensures that follow-up care and community resources are arranged.
- Administers treatments and physical therapy programs to restore function, relieve pain, and prevent disability.
- Performs wound care and assesses the need for specialized equipment in patients' homes.
