Salary :
Summary : Responsible for coordinating, implementing and supervising the effective and efficient use of health and medical related services for the various programs by performing the following duties.
Essential Duties and Responsibilities
- Competent knowledge of regulations, health and wealth disease management, and prevention management as they affect persons in a residential setting.
- Perform initial assessment of new persons and provide clinical recommendation as part of IDT team.
- Knowledge or familiarity with Therap EMR to develop initial assessments and care plans in a timely manner along with ensuring agency documentation is maintained where required, and training of direct care staff is consistent and recorded.
- Develop rapport with persons to identify his / her needs (i.e., self-care skills, social skills, activities of daily living skills, medication management, and mental and somatic health).
- Communicate clearly with all direct care staff regarding the care of the individual, particularly as medical / health events or changes occur.
- Assist with recommendations concerning meals and individual nutritional needs.
- Consult with direct care staff and physicians when health issues arise with persons.
- Document and chart all health issues or findings in a timely manner.
- Be available to assist and respond with staff for persons who may be experiencing mental health, behavioral, and / or medical crisis.
- Develop the agency medical policies to ensure they comply and are up to date with licensing regulations.
- Participate in a monthly team meeting reviews.
- Ability to conduct trainings to aide staff in carrying out their duties
- Must be available to be reached on-call by direct-care staff.
- Knowledge and observation of Privacy and HIPAA regulations for direct care, communications, and documentation.
- Ability to teach TME certification and recertification, and evaluate the performance of TMEs which includes assessment of the knowledge and clinical competency of staff to administer medications.
- Maintain nursing license per state requirements and attend DDA required nursing meetings and trainings.
- Participate in DDA / DDS monitors and have the ability to provide Plan of Corrections when necessary
- Perform any other nursing duties as assigned by the Program Director or the Executive Director.
- Other duties may be assigned.
- Education and / or Experience : Graduate of an accredited school of nursing with a Baccalaureate Degree; Prefer at least two years in a supervisory or administrative position;
experience in community based nursing in one of the following : geriatrics, rehabilitation / restorative nursing, psychiatric nursing or intellectual and / or developmental disabilities.
Supervisory Responsibilities : Provide direct supervision licensed practical nurses.
Certificates, Licenses or Registrations : Unrestricted Registered Nurse license the states in which Project ReDirect operates.
Current CPR Certification; Valid Driver's License and good driving record is required as designated by Project ReDirect Policy and Procedures.

Physical Demands :
Must have ability to travel among assigned sites; must be able to assist with a two-person lift of an adult; must be able to do frequent bending, stooping and lifting;
and must be able to stand for eight (8) hours at a time. Must be able to speak and understand English fluently. Must be able to see and hear or use prosthetics that will enable these senses to function adequately to ensure that the requirements of this position can be fully met.
Must be able to move sufficiently to respond to life threatening emergencies including performing C.P.R. and assisting in the evacuation of residents during emergency situations.
Must be able to lift 50 or more pounds on occasions.
