Job Summary :
The Nurse Navigator functions within the multidisciplinary team as an advocate, , educator, and counselor for referred oncology patients with selected diagnoses, with an emphasis on patients diagnosed with surgical cancer.
The Navigator is responsible for ensuring all patients referred with a cancer related diagnosis or suspicious finding receive effective and comprehensive services.
She / he will coordinate patient care throughout the continuum in collaboration with the multidisciplinary team. She / he will serve as a clinical resource to patients and referring physician offices.
He / she will serve as a liaison throughout the facility and in the community regarding services provided for selected cancer patient populations.
She / he demonstrates broad-based knowledge of cancer care, treatment modalities, resources, data analysis, community services and ability to support patients in navigating the health care processes associated with a cancer diagnosis.
The navigator will provide expert nursing care, which includes : direct clinical practice, consultation, education and research.
He / she will maintain current knowledge with new technology, new treatment modalities and acts as a resource for other clinical staff.
The Nurse Navigator will work as a team with other nurse navigators, cross-covering when appropriate.
Principal Duties and Responsibilities :
The following is a summary of the major functions of this individual's job. He or she may perform other duties, both major and minor, that are not delineated below, and specific functions may change from time to time.
1. The Navigator partners with patients, families, the interdisciplinary team, and community resources to provide well-coordinated, timely, compassionate, , interdisciplinary care.
The Navigator communicates with all members of the healthcare team, as appropriate about patient / family needs and concerns.
2. Utilizes the nursing process to perform assessment of patients clinical, emotional, spiritual, psychosocial needs serving as a single point of contact throughout the care process.
3. Provides education and information to patients and families specific to diagnosis, helping to facilitate seamless, continuous and comprehensive care.
Initiates and documents patient teaching including family and significant others based on assessment of needs.
4. Serves as a resource for community educational events, such as health fairs, screenings, symposiums, and lectures as well as staff education.
5. Coordinates multidisciplinary consults for patients.
6. Develops and / or coordinates development of patient education materials, clinical pathways, and quality studies for identified tumor sites.
7. Coordinates quality monitoring according to treatment guidelines and process improvement initiatives to demonstrate program effectiveness by tumor site.
8. Attends tumor boards and promptly follows up on new referrals.
9. Responsible for outreach efforts to establish and maintain positive working relationships with key customers (physicians, office staff, diagnostic staff, nurses, social workers and radiation oncology staff.
10. Responsible for establishing relationships with surgical providers throughout the region including tertiary / referring medical centers.
11. Tracks and follows patients referred to a non-Baptist facility and assists them to return for further treatment at a Baptist facility.
12. Responsible for establishing a surgical oncology multidisciplinary planning conference with Baptist stake holders and University of Louisville stake holders.
Minimum Education, Training and Experience Required :
A.Bachelor's degree in nursing required.
B.Master's degree preferred.
C.ONCC certification required within 18 months of hire date. Ongoing certification is required.
D.Minimum three years oncology experience required, with a strong knowledge base related to common types of cancers and treatment.
E.Must be able to work as a team member in addition to leadership role in selected projects.
F.Ability to work in an unstructured environment and demonstrate self-direction.
G.Experience in education and training and / or program development preferred, along with excellent written and verbal communication skills.
H.Experience with common computer programs
