Registered Nurse - Primary Care (Bronx)

Position Summary :

We are presently seeking dedicated professionals to join Urban Health Plan (UHP) in the capacity of Registered Nurse I / II.

As an RN at UHP, you help to shape and transform the health and wellness of children, adults, and families that frequent our outpatient primary care and specialty clinics.

You will join a team of professionals and provide quality patient care to individuals of all ages within our Family Medicine Centers located in the Bronx, NY.

Minimum Qualifications :

• Bachelor's Degree in Nursing or Associates Degree in Nursing is required.
• NYS Registered Professional Nurse, required.
• All experience levels are welcome to apply, whether this is your first role as a nurse, or you are coming in as a seasoned nurse.
• Bilingual English / Spanish is required for most openings (or Bilingual English / Bengali).
• Excellent 'bedside' manner, compassion, empathy, and team-driven attributes are essential.
• Knowledge / training in electronic health records and Microsoft Office.

Key Duties :

• Triage patients and assists patients with their needs based on acuity.
• Provides quality nursing care and medication administration. Administers immunizations / vaccines to specified patients.
• Monitors patient reactions and side-effects.
• Assists care team with pre-visit planning for high-risk patients.
• Ensures implementation of plans of care.
• Behaves as a role-model and source of nursing guidance to medical assistants and phlebotomists that are part of the team.
• Assists with prepping patients as needed and providing patient education.
• Assists providers during examinations and treatments.
• Ensures proper sterilization procedures using germicides, sterilizers, or autoclave.
• Ensures departmental adherence to clinical policies and procedures.
• Maintains health records by documenting patient contact and updating patient profile.
• Implements standing orders.
• Checks equipment, logs, and medical kits. Assists with ordering medical supplies, medications, and vaccines.

Starting Compensation :

Registered Nurse I (ASN) : Minimum $81,000 / annum; Maximum $91,000 / annum; Registered Nurse II (BSN) : Minimum $86,000 / annum; Maximum $97,000 / annum.

Work Environment Type, Location, and Schedule :

Onsite, Bronx, NY, Monday to Friday 8 : 00am-5 : 00pm
