Your expertise will guide patients and families through challenging times, providing essential support and resources.

Your Role and Responsibilities

In this dynamic position, you will :

• Conduct comprehensive psychosocial assessments to identify emotional, social, and environmental strengths and challenges.
• Collaborate with interdisciplinary teams to deliver holistic care.
• Develop bereavement plans to support family members and caregivers.
• Educate patients and families, ensuring understanding and overcoming barriers to care.
• Document assessments and interventions accurately in electronic medical records.
• Facilitate communication of self-determined life care decisions.
• Respond to imminent death indicators, providing compassionate support to families.
• Assist with advanced directives and referrals for community resources.

What We’re Looking For

To thrive in this role, you should have :

Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) in Illinois

Bachelor's Degree in Social Work

Working Conditions
