Take on the vital role of developing, implementing, and managing individualized patient care plans within a hospice setting. Collaborate closely with physicians and interdisciplinary teams to optimize patient outcomes and provide compassionate support to patients and their families.
Essential Functions
- Create tailored care plans based on medical and nursing diagnoses for patients ranging from newborns to geriatrics in hospice care.
- Deliver skilled nursing services aligned with patient and family goals, focusing on symptom management and comfort.
- Maintain clear communication with hospice staff, volunteers, physicians, and family members regarding patient assessments and care updates.
- Supervise Home Health Aide services and ensure timely, accurate documentation in accordance with agency policies.
- Foster positive relationships with patients, families, healthcare providers, and referral sources.
- Observe and address patients' physical, emotional, mental, and spiritual needs while adhering to privacy regulations concerning Protected Health Information (PHI).

Graduate of an accredited nursing program.Several years of clinical nursing experience.Preferred : Bachelor of Science in Nursing (BSN), hospice and / or home health experience, and certification as a Hospice and Palliative Care Nurse.Licensure and Certification
