This is an exciting opportunity with Promise Healthcare primarily located at Promise Healthcare-Urbana. The Promise Urbana School Health Navigator (HN) will work closely with school health center clinical teams and serve as a liaison between Promise Health, our patients, and our school partners in carrying out the school health center workplan focused on increasing clinic utilization and providing supportive services and assisting in addressing Social Determinants of Health.

In addition, the Health Navigator will work with Promise Outreach and Marketing staff to increase awareness of health center services and improve the utilization of services.

In addition, the Health Navigator will serve as a liaison between school and public health staff and Promise Health Center staff.

ESSENTIAL DUTIES / RESPONSIBILITIES :

  • Represent Promise Healthcare at school and community events and schedule and enroll patients.
  • Build rapport and connection with patients in an empathetic manner.
  • Crosstrain as a Patient Services Representative and cover for the PSR as needed.
  • Record enabling services provided and patient notes in the Nextgen EMR.
  • Assist patients in scheduling, accessing, and utilizing clinic services. If the patient does not have a PCP, work to get the patient established with a PCP.
  • Screen patients for Social Determinants of Health Issues (SDOH) and make service referrals as appropriate.
  • Participate in the School Clinic and Community Coalition.
  • Assists with grant reporting and needed tracking and record keeping.
  • Assist with outreach to patients and potential patients by calling, emailing, and public speaking.
  • Attend school and community events to promote services.
  • Work with Marketing and Patient Access staff to promote school health center services to increase utilization and promote new services.
  • Follows HIPAA policies and procedures.
  • Assesses patient needs to customize appropriate internal and external resources. Assists families in the community and home to remove barriers to access to services.
  • Work with individuals from diverse backgrounds to reduce cultural and socio-economic barriers between patients and institutions.

Builds family capacity to address health and social issues through education, care coordination, and community outreach.

QUALIFICATIONS :

  • Ability to think critically and make decisions based upon information provided.
  • Must be able to maintain professionalism with patients, co-workers, staff, and referring agencies.
  • Self-Starter and independent worker.
  • Ability to communicate effectively by telephone, in person, and virtually.
  • HS diploma required. Associate degree preferred.
  • Experience with or willingness to train on Medicaid or Marketplace insurance enrollment a plus.
  • Willingness to complete Community Health Worker training.
  • Must be able to attend occasional community events on evenings or weekends.

WORK ENVIRONMENT :

Work is primarily at the Promise Urbana School Clinic school-based health center at 1002 S Race St, Urbana, IL 61801 but the HN may work at other Promise locations in Champaign and Urbana.

This position is a long-term temporary position grant funded through June of 2025 through a long-standing grant. There is a potential for the position to be extended or the HN may transition to another Promise Outreach or PSR role when funding ends.

The work may require some early morning hours, evenings, and weekends to participate in school and community events. This position reports to the Vice President of Strategy and Development.
