POSITION SUMMARY :
The Private Duty RN is responsible and accountable for providing Quality / No Harm, Customer Experience, and Stewardship.
By : Providing and documenting skilled nursing care to patients in accordance with a written plan of treatment prescribed by the physician.
Develops, implements, evaluates and revises written plan of care for each patient. Provides appropriate interdisciplinary case management of patients, demonstrating understanding of cognitive, physical, emotional and chronological.
Primary Accountabilities
- Coordinating delivery of care by other disciplines as appropriate.
- Makes appropriate referrals to other disciplines and demonstrates multidisciplinary collaboration.
- Evaluates patient outcomes and communicates progress and changes in condition to physician.
- Supervises the Home Health Aide.
- Accepts any referral willingly.
- Utilizes supplies in a cost effective manner.
- Assesses for barriers to learning and performs pertinent patient teaching each visit.
- Demonstrates effective problem solving skills.
- Performs complete assessment on admission if requested; interprets data and bases plan of care on assessment findings.
- Implements medical plan of treatment and makes appropriate nursing interventions based on identified needs.
- Develops plan of care in conjunction with Clinical Operations Manager, which includes safety, educational and psychosocial needs.
- Documents in an appropriate and timely manner.
- Complies with State / Federal Regulations and Private Duty policies regarding delivery of home care services.
Qualifications
QUALIFICATIONS REQUIRED :
- Graduate of an accredited school of professional nursing.
- SPECIAL QUALIFICATIONS :
- Current Florida RN license
- Current CPR from American Heart Association
- A current American Safety and Health Institute (ASHI) BLS CPR card for Healthcare Providers will be accepted upon hire.
- However, associate must obtain American Heart Association BLS CPR card within 90 days.
- Successful completion of Level II background screening (fingerprinting).
- Current valid Driver License or ID card.
- Ability to safely operate a motor vehicle as evidenced by driving record for 3 years prior to employment preferred. NOTE : Associates whose driving record does not meet this criteria may limit their ability to work on assignments requiring transportation.
PHYSICAL DEMANDS :
- Exposure to biohazardous waste, chemical substances and unknown diseases.
- Care is provided in a variety of home environments.
- Ability to move in crowded conditions.
- Ability to operate automobile in variable conditions.
- Visual acuity and hand eye coordination to perform clinical tasks.
- Must be able to lift, turn, pull or ambulate patients up to 20 times daily.
- Must be able to lift up to 40 pounds, unassisted, up to six times daily.
- Must be able to speak and understand the English language.
- Must be able to pass colorblind test in order to perform those point of care tests requiring color changes.
- Must be able to stand, walk, bend and stoop for 90% of assigned shift.
