Overview
HarmonyCares is seeking a Registered Nurse Home Health Visiting Nurse to provide intermittent skilled nursing services in a face-to-face capacity to include patient residence and qualifying facilities.
Responsibilities
- Admit patients eligible for home care services within 24-48 hours
- Assess and evaluate patient needs / problems and develop patient care plans
- Provide intermittent Skilled Nursing services including assessment, evaluation, procedures, teaching and training activities
- Supervise Home Health Aide and act as a preceptor in the orientation of new nursing staff
- Participate in Process Improvement (PI) program and discharge planning process
Qualifications
- Current unencumbered State professional Nurse License
- Valid driver's license and good driving record
- Ability to make sound professional clinical judgment
- Proficient clinical skills and excellent verbal and written communication skills
