HOME CARE: LPN FULLTIME

Home Care Licensed Practical Nurse (LPN) : < / Enjoy independence, autonomy, and flexibility in the field to visit patients one on one in their home environment.

Mileage Reimbursement / Company Car Program< / Generous PTO, Holiday Pay, 401k< / Medical, Dental, Vision Insurance,< / 100% Clinical License Reimbursement< / CEU Reimbursement - Up to $400 a year and 2 days of CEU time off< / Full-time Days 8 : 00am - 5 : 00pm< / This position will work in the field helping patients in their home setting.

The office is in Roseville, MN and the general territory for this position would be the Southwest Twin Cities Metro area (the cities that are south of Highway 394 and west of 35W).

Flexible scheduling of patient visits.&Able to travel within geographic area serviced by the office. Participate in the development and evaluation of the plan of care in partnership with the patient, representative, and caregiver(s).

Provides services that are ordered by the physician as indicated in the plan of care.&Counsels and educates the patient and family in meeting healthcare related needs.
