HOURS : FULL TIME - MON-FRI 8AM TO 5PM (FLEXIBLE TO WORK SOME WEEKENDS)

Summary & Objective

The Licensed Practical Nurse Admissions Supports is responsible for supporting referrals, coordinating patient / family needs for admission to the Hospice Program and coordinating with the Interdisciplinary Team members as required.
