Requirements for Director of Social Services (LSW) :
- Must possess a minimum of a B.A in Social Work
- Active Massachusetts licensure as an LSW, LCSW, or LICSW, as required by the MA Department of Public Health
- Skilled Nursing Facility (SNF) experience, preferred
Job Duties of Director of Social Services (LSW) :
- Compiles social history data
- Participates in patient / resident assessments, and the development and evolution of residents' care plan.
- Responsible for planning and leadership interdisciplinary care planning weekly meetings.
- Documents timely progress notes according to state and federal regulations.
- Coordinates the discharge planning for residents, including facilitating discharge planning family meetings, coordination of home-based services to support a successful discharge to home, and ordering of patient-specific durable medical equipment (DME) to home.
- Able to interpret and implement established social service policies, procedures and regulations.
