Resident Care Coordinator- RN or LPN
Canterbury Place , a premier senior living community in Pittsburg, is seeking a compassionate and dedicated Resident Care Coordinator (RCC) to join our team! As an RCC, you will play a vital role in ensuring the well-being and happiness of our residents in our Personal Care Homes (PCH).

Our PCH program provides assisted living services in a comfortable and secure home-like setting.
Responsibilities :
- Oversee the daily care needs of assigned residents, working closely with caregivers and other healthcare professionals
- Develop and implement individualized care plans that promote resident well-being and independence
- Monitor residents' health status and report any changes to nurses
- Assist residents with activities of daily living (ADLs) such as dressing, bathing, and medication management
- Ensure a safe and secure environment for residents
- Staff scheduling, staff oversight, filling in for shifts (if needed), assessing potential residents, etc
- Respond to resident concerns and requests promptly and compassionately
- Maintain accurate and complete resident records
- Supervise Med Techs / NA's
- Flexibility to be able to work the floor / pass meds / assist with care & documentation as needed
- Collaborate with families to keep them informed and involved in their loved one's care
Qualifications :
- Minimum of 2 years of experience working with seniors in a long-term care or assisted living setting
- RN or LPN license in good standing in the State of Pennsylvania
- Strong understanding of senior care best practices
- Excellent communication, interpersonal, and organizational skills
- Compassionate and patient demeanor
- Ability to work effectively as part of a team
- Positive and enthusiastic attitude
