We're currently seeking a qualified Clinical Social Worker to join this group of caring professionals. The ideal candidate will have extensive experience in a patient-oriented medical environment.

Objectives of this role

• Responsible for counseling patients
• Assisting them in gaining access to various resources Assisting patients to confront issues that are negatively affecting their way of living such as poverty, abuse, mental health, and addiction.
• Must be able to exhibit strong interpersonal and communication skills as well as the ability to utilize both internal and external resources to accomplish goals and objectives.

Responsibilities

• Work effectively in a fast-paced primary care environment Displays exceptional interpersonal skills as part of the multi-disciplinary personnel team.
• Assess, diagnose, treat, and develop treatment plans for patient(s) with mental, emotional, and behavioral issues.
• Provide patients with self-management skills and educational information needed so they can be full participants in their own treatment.
• Collaborates with the provider and the patient.
• Work collaboratively with a wide range of clinical and non-clinical disciplines to assess patient and family needs, design effective care planning or service goals or medical treatment plans regarding utilization of additional resources.
• Prepare and maintain complex treatment and progress reports and document patient medical charts as needed which include diagnostic and treatment recommendations, discharge planning and follow-up services.
• Participate in crisis intervention as needed.
• Complete all necessary documentation for billing requirements.
• Research, and otherwise gather information regarding community resources, including pharmaceutical resources for patients needing assistance in paying for copayments.
• Demonstrate multicultural skills, knowledge, and experience, including the influence of cultural issues and providing services with appropriate cultural context.
• Advise patients of available community resources and act as a liaison and / or advocate for patients / families to use the available services / resources effectively.
• Performs other duties and responsibilities as assigned or requested.

Required Skills and Qualifications

• Masters in Social Work
• One to two years related experience and / or training as a Clinical Social Worker Relating in a professional manner and handling confidential information and protected health information.
• A high degree of initiative, judgement, discretion, and decision making
• Excellent communication and organizational skills Working knowledge of patient management software systems
• Proven ability to collaborate effectively with colleagues
• Preparing clear concise casework records; make recommendations based on information; organize and manage caseload
• Analyzing situations accurately and taking effective action
• Good organizational skills Proper spelling and use of medical terminology Proficient in use of Microsoft Office Suite and electronic medical / health records.
