A company is looking for a Remote Behavioral Health Care Manager (HIE) to manage care for members following Emergency Department visits for behavioral health or substance use issues.
Key ResponsibilitiesAdvocate for and educate members on services and self-management techniquesConduct assessments to identify barriers and generate referrals to appropriate resourcesCoordinate care with multidisciplinary teams to ensure person-centered treatmentRequired QualificationsNYS RN or LCSW, LMSW, LMFT, LMHC, LPC, or licensed psychologist (any state)Minimum of 3 years of experience in a mental / behavioral health or addictions settingExperience managing member information in a shared network environmentAbility to manage large caseloads in a fast-paced environmentProficiency in Microsoft Excel and electronic documentation systems
