About The Position
Job Title : Rehabilitation Therapy Specialist Job Specialty : Physical Therapy Assistant Job Duration : 13 Weeks Shift : Monday-Friday, Day Shift Guaranteed Hours : 32 Hours Per Week Experience : New Graduates Welcome;
Skilled Nursing Facility Experience Preferred License : State License Required Certifications : CPR Certification Must-Have : Completion of Prescreen Form- Submission of Resume and License Verification- Covid Test 72 hours Prior to Start Date with Results Issued 48 hours Prior Job Description : Provide assistance in physical therapy to patients under the supervision of a licensed physical therapist.
- Conduct exercises and treatments according to the plan established by the supervising therapist.- Monitor patient progress and report findings to the supervising therapist.
- Maintain patient records and ensure documentation is complete and accurate.- Ensure compliance with professional practices and standards as well as policies and procedures.
- Attend necessary training and orientation sessions to align with facility protocols.- Perform other related duties as assigned to support patient care delivery and therapeutic objectives.
Credentialing Checklist : State License Verification- Resume and Three References- County, Federal, Multi-State Criminal Background Checks (7 years)- Nationwide Sex Offender Search (7 years)- HHS / OIG and GSA Search- Social Security Number Trace- 1 Step PPD or Quantiferon or Proof of Positive PPD and Chest X-Ray- SAM Search- Physical Examination Position Urgency : Rapid Response Crisis
