How you'll contribute

A Case Manager (RN) who excels in this role :

• Responsible for overall management of the care coordination of observation / inpatients in collaboration with the care team.
• Also responsible for appropriateness of care and resource utilization, as well as, continued stay reviews
• Responsible for providing education as an adjunct to care team, particularly in regards to chronic disease management.
