We are looking for a Registered Nurse to join our growing organization! Our ideal candidate will have 2-5 years of Homecare Experience and be familiar with the Department of Health Regulations .
Education and Experience :
- Bachelor's Degree and three years of related work experience strongly preferred
- Current Registered Nurse Certificate from the NYS Department of Health
Primary Responsibilities (including, but not limited to) :
- Ensure to complete all documentation for nursing assessments, observations and planning evaluation pertaining to clients.
- Evaluate clients on a regular basis and / or as necessary to ensure clients needs are consistent with the care plan
- Utilize professional assessment skills to identify physical, mental and psychosocial needs of the client and be able to develop a care plan to meet these needs in a multi-disciplinary approach
- Act on client’s behalf to assure patient’s rights
- Be aware of home safety issues, strive to accomplish all responsibilities in a safe manner and consistently reinforce good safety attitudes and practices displayed by other staff members
- Respond promptly to any client complaints concerning a Field Staff member, document complaints, review complaints with Case Management and take appropriate action with the Field Staff.
- Conduct client care reviews and visits to ensure quality of care
- Assist with client schedules and updating all client care plans
- Provide on going In-service training to all field staff as well as conduct observations and evaluation of personal care services
- Review care plans and medical orders to reflect priority needs of client
- Conducting caregiver observations, file compliance to ensure all caregiver physicals are up to date and ensuring all annual evaluations for caregivers
- Assist with service inquiries, assessments and client acquisition protocol
- Ensure all Implementation of care is being done as outlined by assessments, care plans, Department of Health and Senior Helpers regulations.
- Maintain ongoing communication with clients and their families to ensure that they are satisfied with Senior Helpers services
- Perform Case Management on-call

- Supervise (instruct, assign work, etc.) caregivers
- Excellent verbal and communication skills, adapt to different situations and possess excellent client interaction skills
- Multi-task and work independently.
