Home Care Registered Nurse (RN) - Aurora

Maxim Healthcare is seeking a compassionate and experienced Registered Nurse (RN) to join our home health care team in Aurora.

You will provide high-quality care to pediatric and / or adult patients in their own homes, making a positive impact on their lives.

Responsibilities : + Implement and evaluate individualized care plans for contract care patients, considering their unique needs and goals.

• Collaborates with healthcare providers to regularly review and adjust care plans to address evolving patient needs and ensure optimal outcomes.
• Provides comprehensive care that includes therapeutic, preventive, and rehabilitative interventions to promote patient recovery and well-being.
• Monitors patient's condition closely and promptly reports any significant changes to the appropriate healthcare team members.
• Ensures accurate and timely medication administration, monitoring treatment effectiveness and documenting outcomes. + Continuously assesses care plans and identifies opportunities for patient education, care coordination, goal setting, and discharge planning to support optimal patient outcomes.

Requirements : + Current Registered Nurse (RN) license in the state of Colorado. + Current Health Certificate (as required by facility or state regulations).
