Role Overview :
Join our client as a Clinical Social Worker in Long Island City, where you will perform mental health assessments, provide individual and group counseling, and collaborate with a team to ensure high-quality patient care.
The ideal candidate will have an unrestricted New York State Clinical Social Worker license and a minimum of two years of clinical experience.
Compensation : $80,000.00
Location : Long Island City, New York

Responsibilities :
149; Performing mental health assessments to determine diagnoses and establish treatment plans
149; Providing individual and group counseling and psychotherapy, as well as crisis intervention and conflict resolution
149; Completing medical forms and documentation related to patient care, including facilitating transfers and referrals to outside programs or psychiatric clinics
149; Making and tracking referrals to other mental health agencies and providers, and collaborating with specialists for consultation and referral purposes
149; Working with the intake team to determine the best course of action for each patient
149; Maintaining confidential case files and following clinical chart review policies and procedures
149; Providing workshops for families on communication and other family-related issues
149; Coordinating emergency responses, including minimizing patient's danger to self and others, communicating with EMS / FDNY, and facilitating referrals to psychiatric or medical emergency departments
149; Directing the course of the patient's mental health care and treatment plan within the scope of expertise
149; Encouraging health promotion, disease prevention, and health maintenance, and providing patient advocacy
149; Collaborating with Registration, Scheduling, and Medical Records departments to ensure appropriate patient scheduling, flow, and documentation procedures
149; Observing, implementing, and promoting federal regulations as defined by HIPAA and The Floating Hospital's policies and procedures
Qualifications :
149; Unrestricted New York State Clinical Social Worker license
149; Minimum of two years' clinical experience preferred
149; Familiarity with word processing and other computer applications
149; Ability to speak Spanish and / or other languages preferred
