Responsibilities :
- Assess nature of clients' situations by interviewing and reviewing personal history'
- Conduct patient assessments to identify social, emotional, and environmental needs
- Determine appropriate milestones to gauge client progress
- Develop and implement individualized care plans in collaboration with the healthcare team
- Assist patients in accessing community resources and support services
- Advocate for patients' rights and facilitate communication between patients, families, and healthcare providers
- Maintain accurate medical documentation in compliance with regulatory standards.

- Strong interpersonal skills and the ability to work effectively with multidisciplinary teams

Requirements :
- Master's degree in social work from an accredited institution
