Key Responsibilities
Physicians provide primary and / or specialty medical care in a variety of ambulatory, home and long-term care settings. The Physician is responsible for providing medical leadership to advanced practice providers such as Nurse Practitioners and Physician Assistants. The Physician will function in collaboration with providers according to a Collaboration Arrangement. The Physician's practice will focus on caring for the seriously ill population while providing advanced primary care capabilities to assist patients and families with comfort and quality of life.
In this role you will be responsible for :
Conducts medical evaluations of predominantly elderly, frail, seriously ill patients in their home environment.Performs independently under stress and in emergency situations.Drives between locations, navigating to each location using GPS device or other tools.Communicates significant findings, problems, and changes in condition or environment to the clinical coordinator, family, facility and other personnel involved with patient care.Assess health status by obtaining relevant health and medical history. Performs a physical exam based upon the age and patient history. Identifies significant health and medical risk factors.Orders, interprets and evaluates laboratory tests and other diagnostic studies to assess patient's clinical problems and health care needs. Develops a diagnosis by analyzing and synthesizing collected dataPerforms diagnostic of therapeutic interventions consistent with certification, law and state medical privileges. May prescribe and monitor medications as allowed by prescriptive authority through the appropriate State Board Agency.Develops a treatment plan in collaboration with the patient and significant others to include Medicare, CMS, Ancillary services, and treatment. Collaborates with nursing, medical and other healthcare team members to ensure quality patient care.Refers patients to providers as appropriate for consultation or for specialized health resources and treatment. Ensures continuity of care by serving as a liaison between patient and other members of the multidisciplinary care team or with other specialty areas as necessary.Supervise nurse practitioners and physician assistants per the Collaborative Agreement.Required Qualifications
Doctor of Medicine or Osteopathy requiredCurrent unrestricted State license to practice medicine.Prior supervisor / management experience.Requires demonstrated interpersonal, supervisory, and management skills with ability to communicate effectivelyDemonstrates proven organizational development and project managementExhibits working knowledge of regulations and reimbursement in home health care as well as proven leadership abilitiesSit, stand, bend, lift, and move intermittentlyMust be able to lift at least 50lbs and bear the weight of an average adult effectively Current and valid driver's licenseReliable transportation and valid automobile insurance coverageDesired Qualifications
Board certification in family practice, internal medicine, geriatric or palliative care preferred.Preferred 2 years experience in healthcare, with a combination of acute care, geriatric and / or long-term care practice.Experience with geriatric patients preferred.Familiarity with Aprima EMRLocation
