Job Summary :
The Providence Center Crisis Stabilization Unit provides short-term care for patients experiencing an acute psychiatric and / or substance use crisis that could escalate to a point of requiring hospitalization.
As an alternative to hospitalization or as a step-down for a transition between levels of care, the CSU’s services focus on stabilizing clients and addresses their mental health and substance use needs.
The CSU Case Manager will coordinate services for consumers to minimize stressors and to facilitate the reduction of psychiatric and substance abuse disorder symptoms.
Duties and Responsibilities :
Encourage consumers to attain the highest possible levels of independence.
Make assessments and provide appropriate crisis interventions and risk management as needed.
Teach consumers adult daily living skills.
Initiate treatment and advocate for consumer within network of care.
Negotiate on behalf of the consumer, as needed, with landlords, courts, families, the treatment team, and consumers themselves.
Transport consumers in Program van to planned activities and appointments.
Complete recordkeeping in accordance with Health Information Services (HIS) and other center requirements within required time frames and abide by center confidentiality policy.
Maintain cooperative relationships with Center staff, consumers, community agencies and the public.
Attend trainings, case presentations and conferences.
Requirements :
Associate's degree required, Bachelor’s degree in Human Services preferred.
Experience in Human Services Field preferred.
Must have valid driver's license and insured auto.
