Position Summary

The Home Health Director of Clinical and Operations Solutions is responsible for modeling the Compassus values of Compassion, Integrity, Excellence, Teamwork, and Innovation and for promoting the Compassus philosophy, using the 6 Pillars of Success as the foundation.

S / he is responsible for upholding the Code of Ethical Conduct and for promoting positive working relationships within the company, among all departments, and all external stakeholders.

The Home Health Director of Clinical and Operations Solutions is a multidisciplinary professional with flexible skill sets who can perform effectively as an executive director, clinical director, or member of the EMR deployment and support team.

Primarily, the Home Health Director of Clinical and Operations Solutions is part of a unit of subject matter experts in the organizational EMR system and participates in the development of training materials, deployment of system implementation / conversion, and provision of direct training and support for new users at the individual and program level.

S / he is also engaged as a member of the corporate team at the regional level to perform interim roles based on program-level needs which may include leadership, operational performance management, clinical direction and leadership, and program gap analysis or problem-solving.

Position Specific Responsibilities

  • Oversees the collaborative process between members of the Interdisciplinary Team (IDT).
  • Ensures home health services are available 24 / 7.
  • Serves as agency Administrator as appointed.
  • Manages all members of the IDT, as required, including aspects of scheduling, productivity, mentoring, pay practices, timekeeping, performance reviews, and team member support.
  • Actively collaborates regarding QAPI activities and provides feedback and input into Performance Improvement.
  • Upholds a personal philosophy of integrity and commitment; communicates proactively when schedule changes are needed.
  • Participates in care delivery for patients as needed including conducting admissions, recertification visits, routine visits, and discharge visits.
  • Manages the care for a caseload of patients if needed to support ebb and flow of census changes.
  • Develop and deliver comprehensive training programs for new team members, covering clinical protocols, operational procedures, and EMR system usage.
  • Provides operational support, clinical direction, and leadership as needed to address program-level challenges.
  • Ensures accurate and timely documentation in the patient's clinical record as required, demonstrating proficiency with documentation requirements.
  • Analyzes and reports on quality metrics, including HHCAHPS and Home Health Quality outcomes, for both existing and new programs.
  • Participates in after-hours shifts, including primary and back-up on-call responsibilities, as needed for both existing and de novo locations.
  • Serves as the administrator on call for scheduled periods.
  • De Novo Expansion Responsibilities : Ensures compliance with state regulatory requirements for the supervision of nursing services in existing and newly established de novo locations.

Leads the strategic planning and execution of the buildout process for new home health locations.Oversees the logistical aspects of establishing de novo locations, including facility setup, equipment procurement, and regulatory compliance.

Recruits, hires, and trains a dedicated team for each de novo location, ensuring the selection of qualified and committed staff.

Provides ongoing leadership and mentorship to de novo teams, fostering a positive and collaborative work environment.Collaborates with regulatory bodies to obtain necessary licenses and certifications for the de novo programs.

Supports ongoing education and skill development to enhance the capabilities of the de novo teams.Conducts regular gap analysis for the de novo programs, identifying areas for improvement and implementing solutions.

Performs other duties as assigned.

Education and / or Experience

  • Bachelor’s degree is strongly preferred but not required.
  • Minimum of five (5) years of nursing experience, with three (3) years in home health required.
  • Minimum of five (5) years of management experience, with two (2) years in hospice or home care required.
  • Proficient in the use and function of Homecare Homebase (HCHB) EMR for hospice or home health experience must be in the past 12 months.
  • Experience in a regional role in hospice or home care is preferred but not required.

Skills

Mathematical Skills : Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.

Ability to compute rate, ratio, and percentage.

Language Skills : Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.

Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties.

Strong written and verbal communications.

Other Skills and Abilities : Ability to articulate home health roles for the IDT and apply principles of care delivery to use of EMR system.

Ability to understand, read, write, and speak English. Articulates and embraces integrated healthcare at home philosophy.

Certifications, Licenses, and Registrations

Active and unencumbered Registered Nurse or Physical Therapist license or COMPACT license is preferred.

Physical Demands and Work Environment : The demands of this role necessitate a team member to effectively perform essential functions.

Adaptations can be made to accommodate team members with disabilities. Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 50 pounds.

Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus.

In a healthcare setting, exposure to bodily fluids, infectious diseases, and conditions typical to the field is expected.
