Position Overview

Title : Outreach Specialist

Department : Treatment Services

Reports to : Clinical Supervisor

Essential Job Functions :

  • Track and Monitor all Referral and Intakes
  • Adhere to agency standards of required documentation including assessments, consents, screening tools, progress notes and any additional intake documentation
  • Participate in staff meetings, case conferences, utilization review, staff development, clinical supervision and trainings
  • Assess all new admissions for Medicaid eligibility while maintaining compliance with billing targets and Medicaid documentation
  • Identification of any problems in daily living skills; hands on assistance and / or referral in addressing problems in daily living skills.
  • Liaison to community based services and groups including civic, social service government, and business organizations.
  • Other duties as assigned by supervisor
  • Travel may be required.

Qualifications

Educational Requirements :

  • Clinical Licensure-LMSW
  • Working knowledge of various computer programs including e-mail, Microsoft Word, Excel, PowerPoint and Windows Operating System.

Experience Requirements :

  • Two years’ experience with individuals with mental illness, substance abuse issues and / or a history of homelessness.
  • Bilingual English / Spanish a plus.
  • The ideal candidate will be creative, innovative, open to change and willing to contribute new ideas.
