About The Position
Job Title : Registered Nurse I Job Specialty : Home Health Job Duration : 13 weeks Shift : Day 5x8-Hour (08 : 00 - 17 : 00) Guaranteed Hours : Not guaranteed Experience : Minimum 1 full year Adult Home Health or Hospice experience License : Must have a valid nursing license in hand Certifications : CPR, BLS Must-Have : Proficiency in OASIS and HomeCare HomeBase EMR systems within the last year- Reliable transportation with a current driver's license and liability insurance- Willingness to travel up to 75 miles from the home clinic Job Description : Coordinate and direct the provision of home nursing care according to physicians' orders and agency policies.
- Supervise and evaluate Home Health Aides and Licensed Practical / Vocational Nurses.- Conduct home visits and periodic office meetings.
- Document and travel between 5-10 patient visits per day, managing cases such as Start of Care, Wound Care, and Follow-up Visits.
- Engage in on-call rotations as required, including weekends once every 5-7 weeks.- Cover the designated regional territory, with an expectation of traveling approximately 150-200 miles per week.
- Attend to productivity expectations by achieving 30 points per week through various types of visits and activities.
