Job Overview : We are looking for a Certified Nursing Assistant (CNA / NAR) to join our team! This role is responsible for providing hands on care and support services for our newest Skilled Nursing Community (TCU / LTC) in downtown St. Paul, Minnesota. This direct care schedule includes; + 32 hours every two weeks + Day shift; 6AM - 2 : 30PM + Every other weekend and holiday rotation

Ebenezer Integrated Care & Rehab (EICR) serves seniors with long-term, transitional, and dementia senior care needs in the heart of downtown St. Paul. As part of Fairview's Community Health and Wellness Hub, EICR is committed to delivering innovative, collaborative care. The Fairview Community Health and Wellness Hub opened in 2022 to deliver a variety of services to the communityfrom primary and mental health care to enrichment options for seniors, food access programs, and community gathering spaces. The Hub brings Fairview services together with local organizationsincluding Minnesota Community Care and Sanneh Foundationto make it easier for people to access what they need to be well.

Job Responsibilities :

• Assists resident with personal cares based on individual plan of care (bathing, oral hygiene, grooming, toileting, nail care, dressing, showers and answers and responds to call lights)
• Support patients by helping to turn, lift or transfer
• Observes residents and reports status to appropriate staff in a clear, concise manner
• Chart or document in electronic medical record system
• Seeks to resolve resident and family concerns
• Receives and gives report to appropriate staff during various times in work shift
• Performs environmental and other responsibilities as assigned such as; order and cleanliness in rooms and work areas, reports malfunctioning equipment to maintenance department, and answers phones in a timely manner.
• May check and record vitals, per facility practice
