A New York City-based healthcare network is currently seeking an experienced, licensed Registered Nurse (RN) to join their Home Care team in Queens as a Nurse Team Leader.
In this role, the Nurse Team Leader (RN) will be primarily responsible for coordinating, implementing and continually evaluating the home care needs of patients.
Responsibilities :
The Nurse Team Leader (RN) will :
- Precept new nurses and for supervise peers on their field visits
- Review patient insurance information
- Coordinate activities involved in each individual care plan
- Make sure that pertinent findings are shared with family and professional caregivers
- Perform other duties, as needed
Qualifications
- 2+ years of previous Home Care experience
- Bachelor's Degree in Nursing
- NYS Registered Nurse (RN) license
- Solid assessment and documentation skills
- Patient oriented
- Great interpersonal skills
- Excellent communication skills (written and verbal)
- Strong attention to detail
- Highly organized

- OASIS certification
- Previous exeprience in a lead role
