Family Primary Care Nurse Practitioner, Community Health Center, N. California
A non-profit network of community-based Federally Qualified Health Centers (FQHC) offering comprehensive medical, dental, behavioral health, pediatrics, chiropractic, and specialty healthcare services is seeking a Primary Care Nurse Practitioner.
The nurse practitioner provides primary clinical care of clients of all ages. Clinical care includes obtaining health history, performing physical examinations, assessing, and managing physical and psychosocial problems.
Developing a problem list, a diagnostic, therapeutic and teaching plan, which includes preventive health. Provides patient education and counseling.
Offers family planning and STD counseling and services. Furnishes and adjusts medications in the formulary in accordance with the law.
Orders diagnostic studies such as laboratory and radiologic tests, as needed. Refers patients for specialty evaluation and care as needed.
Reviews patients records, including laboratory data, for abnormalities and completeness and is responsible for providing appropriate follow-up.
Precepts and teaches nurse practitioner students and other nursing personnel. Monday through Friday schedule. No call, No Nights and No weekends.
