Weare seeking Full-time Primary Care Physicians for our Outpatient clinic in Rockford, IL. The schedule is Monday-Friday 8a-5p; no nights and no weekends.

RequiredQualifications :
- Nationalcertification in at least one of the following specialties : Internal Medicineor Family Medicine
- Active,non-probationary state physician License
- 1+year direct primary care experience as a Physician
- ActiveDEA license
RoleSummary :
- Provisionof exceptional primary care
- Conductoffice visits for routine and acute issues, and home visits as needed
- AdministerAnnual wellness visits and health risk assessments
- Carecoordination with other providers, specialists, testing facilities, andagencies
- Populationhealth leadership, in coordination with the Care Team
- Assistingthe care team with phone triage and outreach
- Educatingpatients on their health conditions, care plans, and treatments
