Position Description
Lead multidisciplinary treatment team in developing a treatment plan for each patient and documents information in progress notes.
Provides individual therapy according to treatment plan.
- Provides group counseling sessions.
- Assures coverage of scheduled activities such as educational and therapeutic groups.
- Coordinates the development of a therapeutic milieu through an honest, supportive and structured atmosphere.
- Keeps informed of community treatment resources from referrals to and from the program for patients and their families as evidenced by listing service for mental health, group home, assistance programs, -step programs and DCFS
Participates in program planning (curriculum, program changes, etc.)

Qualifications
Job requirements section :
Education : Bachelor’s degree in the human services field (counseling, psychology, social work).
Experience : Previous professional experience in a behavioral health setting preferred.
Other knowledge / skills : Ability to work successfully as a member of an interdisciplinary health care team and the ability to function independently when needed.
